Correction of Inverted Nipple Using APTOS Threads

Abstract
Various methods have been reported to resolve nipple inversion. However, some surgical procedures involving the purse- string suture technique with monofilaments have the disadvantages of loosening, over-tightening, and of allowing recurrence. To lessen these possibilities and to facilitate monitoring postoperatively, we used an APTOS thread as a purse-string suture material for all grades of inverted nipple. Eight inverted nipples in 6 patients were treated from Dec. 2003 to Oct. 2004. Two of these nipples were grade I, three nipples grade II, and three nipples grade III. Grade I nipples were managed using an APTOS purse-string suture only through slit incision. After surgical release of fibrotic bands in the grade II group, an APTOS thread purse-string suture was placed at the nipple base. In grade III, the fibrotic bands were widely dissected and central portions of the lactiferous ducts were cut. Dermal flaps were used to make up for any soft-tissue deficiency, and again an APTOS thread was also used. No major complications occurred except a single grade II nipple revision due to underprojection. APTOS threads are expensive but their characteristics make them useful as purse- string suture materials for inverted nipple correction.
